Membership Renewal (Qatar-based Members)

Steps to renew PHASE Membership

  1.  Download the Renewal Form and Code of Conduct.

  2.  Fill in the form clearly.  Make sure all the information asked is provided.

  3.  Bring it with you at the venue of PHASE General Monthly Meeting or during a scheduled seminar.

  5.  Submit both the Renewal Form and signed Code of Conduct to the Membership Acceptance Desk. 

  6.  Wait for the processing and approval.  Please note that renewal requires at least six (6) PHASE CSEPs or its

      equivalent. 

  7.  Pay membership application fee.  You will be given a claim stub which you need to present upon claiming    

       your renewed ID.

  8.  Make sure a picture of you have been taken to update the database.

Membership Renewal (non-Qatar-based Members)

By virtue of a Board Resolution recently approved by the PHASE Body, non-Qatar based members can now renew their membership.

  1.  Download the Renewal Form and Code of Conduct.

  2.  Fill in the form clearly.  Make sure all the information asked is provided.

  3.  Scan and email the following to phaseqatar10@gmail.com:

       3.1. Renewal Form

       3.2. Signed Code of Conduct

       3.3  Latest photo

       3.4  Proof of continued safety education taken within the recent year (renewal requires safety CSEPs        

       equivalent to six (6) PHASE CSEPs).

  4.  If your renewal application is approved, you will receive a notification plus details of payment procedure.

  5.  Once proof of payment is received, you will receive a notification that renewal process is finalized.  Original

       copy of your ID will be mailed to you.
